Hi, I'm Evangeline "Angel" de Guzman, DNP, FNP-C, WCP-C, CSWD-C — a board-certified Primary Care Provider in Family Practice and Wound Specialist, and the founder of Agape Medical Services.
With over 15 years of healthcare experience, my passion has always been caring for people not just as patients, but as family. I created Agape Medical Services with a vision of providing compassionate, personalized, and Christ-centered healthcare where every person feels heard, valued, and genuinely cared for.
Throughout my healthcare journey, I have had the privilege of serving patients in primary care, wound care, home health, long-term care, rehabilitation, and mobile healthcare settings. My clinical focus includes chronic disease management, preventive care, diabetes, hypertension, cardiovascular health, wellness care, and advanced wound treatment. I am passionate about helping patients improve their quality of life through education, compassionate support, and individualized care plans tailored to their unique needs.
As a Doctor of Nursing Practice (DNP)-prepared provider, I believe healthcare should go beyond simply treating symptoms. My goal is to care for the whole person — body, mind, and spirit — while building meaningful relationships grounded in trust, compassion, and integrity.
My faith is at the center of everything I do, and I strive to reflect God's love through serving others with excellence, kindness, and genuine compassion. Hi, I'm Kristine Alarcon, Medical Assistant at Agape Medical Services.
My role is to support our provider, Angel, and ensure that every patient who walks through our doors feels welcomed, comfortable, and well taken care of from the moment they arrive. I'm here to make sure visits run smoothly — from rooming patients and taking vitals to assisting with procedures and following up on care needs.
I take a lot of pride in the behind-the-scenes work that makes great patient care possible. Whether I'm preparing the exam room, coordinating with the provider, or simply checking in on a patient, I want every interaction to reflect the same warmth and compassion that Agape Medical Services stands for.
Driven by faith and a servant's heart, I don't see my work as just tasks to complete — I see it as an opportunity to make someone's day a little easier and their experience a little brighter. Supporting Angel in delivering exceptional care is something I take seriously and feel genuinely called to do.

Health Sharing Alternative
Impact Health Sharing is a non-profit community health sharing program — not traditional insurance. Members contribute monthly to a shared pool that covers each other's eligible medical expenses, cutting out the middlemen and reducing costs by up to 50%.
As a Direct Primary Care provider, Agape Medical Services is fully compatible with Impact Health Sharing. In fact, your DPC membership costs may even count toward your Wellness Rewards, reducing your annual responsibility amount.
Select a membership tier based on your household size and the Annual Primary Responsibility Amount (PRA) that fits your budget.
Make a low monthly contribution — up to 50% less than traditional insurance — that goes directly to covering other members' medical needs.
When you have an eligible medical expense, the community covers 90% of costs after your PRA is met, with a maximum co-share cap of $5,000/year.
